I'm having trouble with understanding what is the correct grammar rule regarding articles for the next sentence:

A request, payload or attachment was not found.

  1. Do we need to repeat indefinite article before each of noun like this:

A request, a payload or an attachment was not found.

  1. Do we need to use were instead of was in this case?

Each item in "an A, B, or C" type construction is being considered singly. We do not need to repeat the indefinite article before each one, and the verb form of 'be' will be singular. – Michael Harvey Jan 9 at 17:45
